In consideration of my application and permitting me to participate in the 2026 SUGARLOAF MARINA THURSDAY NIGHT WALLEYE LEAGUE being held on Thursday evenings and running from Thursday June 11th, 2026 through Thursday August 27th, 2026, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:

(A)   Waive, release, and discharge from any and all liability for my death, disability, personal injury, property damage, property theft or actions of any kind which may hereafter accrue to me while participating in or travelling to and from this event, Jordan Bates, BUDWEISER (Labatt Breweries of Canada), Whiskey & Walleye, Erie Tracker Outfitters, and their directors, officers, employees, volunteers, representatives and agents, the event organizers, sponsors and event volunteers,

(B)   Indemnify and hold harmless all entities or persons mentioned in this paragraph from any and all liabilities or claims made by other individuals or entities as a result of my actions during this event.

It is the Team Captain's responsibility to ensure that all individuals fishing as part of his team on any given week have read the Rules & Regulations for the Thursday Night Walleye League and have signed this Waiver and Release form.
